An agreement with Microsite Health for the CRTX software solution begins on the date of execution of the Agreement and continues monthly, renewing automatically on the same date of the subsequent month.
You may terminate this agreement at any time AFTER completing the initial 6-month minimum contract term by providing 30 days’ advance notice by clicking this Cancellation Form link below and completing the Cancellation Form:
The termination will take effect 30 days after Microsite Health’s receipt of such termination notice. The Cancellation Form is the only method for canceling the service.
If the next monthly billing cycle falls within the 30-day termination notice period, the dental practice will be charged and responsible for payment of the final monthly fee. Upon the effective date of termination, all CRTX services, access and usage rights will be discontinued. Monthly payment obligations are not prorated. All fees paid for CRTX services delivered prior to the termination date are nonrefundable. After the effective termination date, no further monthly fees will be due.
IF USING THE SALES CENTER: You may terminate the sales center portion of your agreement at any time AFTER the 3-month minimum requirement, and all services will stop at the end of the monthly cycle. The monthly fee is not prorated.
EARLY CANCELLATION
Cancellation requests received prior to the end of the initial 6-month term will effectively turn off automatic renewal, so CRTX will not renew after the first 6 months. However, you will still owe any remaining payments for that initial 6-month term.
